What “Difficult” Means in a Criterion-Referenced Exam
The most useful thing to understand about GESEA Level 1 is that it is criterion-referenced. There is no quota of passes, no ranking against the cohort, and no examiner discretion to round a performance up. Each component — the theory examination and each psychomotor station — has defined benchmarks, and a candidate either meets them or does not. Two consequences follow: nobody in the room is your competitor, and the standard does not move.
That structure changes what difficulty means. In a curve-graded exam, difficulty depends partly on the field; here it depends almost entirely on the distance between your current skill and a fixed, published standard. A candidate who has done the prerequisite e-learning carefully and accumulated deliberate simulator practice generally finds the day demanding but fair; a candidate relying on operating-theatre experience alone often finds the timed, abstract tasks less forgiving than expected.
It also changes how to interpret nerves. Because the benchmarks measure accuracy and economy of time on standardised tasks, performance anxiety expresses itself as rushed, imprecise movement — exactly what the scoring detects. Preparation that includes timed runs under self-imposed pressure is therefore not a luxury; it is training the specific condition the assessment creates.
The Four Components, and What Each Demands
Level 1 combines one theoretical examination with three validated psychomotor stations, and they demand genuinely different preparation. The theory examination, TESTT, tests the knowledge base built in the mandatory e-learning: instrumentation, energy sources, safe entry and intra-operative safety principles. It rewards systematic study of the provided material rather than general surgical reading.
The laparoscopic stations split the manual skill set in two. LASTT isolates camera navigation, hand–eye coordination and bimanual tissue handling under a fixed camera logic; it punishes wasted movement more than slowness. SUTT assesses laparoscopic suturing and intracorporeal knot-tying, where precision of needle handling and the integrity of the finished knot are scored alongside time — for most candidates this is where the learning curve is steepest and where practice hours pay the highest return.
HYSTT moves to the hysteroscopic axis: navigating a scope within a confined cavity, keeping orientation, and targeting accurately under timed conditions. It feels unfamiliar even to candidates comfortable with laparoscopic tasks, because the instrument logic — a single working axis viewed at an angle — is different. Treating it as a distinct skill to rehearse, rather than a variation on LASTT, is the practical adjustment.

Building a Home Practice Routine
The psychomotor foundations of Level 1 can be rehearsed away from any accredited venue. A basic box trainer — commercial or self-assembled, with a fixed camera, two ports and standard graspers plus a needle holder — supports meaningful practice of coordination, transfer and suturing tasks. The validated station equipment on the day will differ in detail, but the underlying skills of instrument triangulation, depth judgement under 2D vision and controlled needle work transfer directly.
Structure matters more than volume. Short, frequent, focused sessions beat occasional marathons: motor learning consolidates between sessions, and fatigue degrades exactly the precision being trained. A workable pattern is twenty to forty minutes several times a week, each session split between one coordination task and one suturing task, always finishing with a timed, uninterrupted run recorded against your own previous times.
Sequence the preparation deliberately. Complete the e-learning early, because its content explains what the stations measure and why; practise untimed until movements are consistent, then add time pressure; and in the final weeks, rehearse the full sequence of tasks in one sitting to build the stamina of sustained precision. Candidates who arrive having simulated the shape of the day, not just its tasks, give themselves the calmest possible version of it.
On the Day, and Reading a Result That Falls Short
The day itself rewards ordinary examination discipline: arrive with the prerequisites verifiably complete, listen to the station briefings — the permitted time and scoring logic are stated, not secret — and treat each station as a fresh event. Because scoring is per component, a rough patch at one station has no arithmetic effect on the others; the costliest mistake is carrying frustration from one task into the next.
A component that does not meet the benchmark is information, not a verdict. The criterion-referenced design means the result identifies precisely which skill needs more consolidation, and a further attempt can normally be booked at a future official session under the applicable GESEA rules, charged separately. Candidates who return after targeted practice are retaking a defined task they now understand, which is a very different proposition from the first attempt.
It is also worth keeping the credential in proportion. Level 1 documents that defined knowledge and psychomotor standards were met under assessment at a point in time — a foundation for supervised clinical progression, not a finish line. Preparing for it properly builds habits, particularly deliberate practice and honest self-timing, that outlast the certificate itself.
