What does this involve?

  • Wisdom tooth extraction is the surgical removal of one or more third molars — the rearmost teeth in each quadrant — indicated where the tooth is impacted, recurrently infected, or causing damage to adjacent structures. The procedure ranges from a routine forceps extraction of a fully erupted third molar to a surgical extraction of an impacted tooth requiring flap elevation, bone removal, and tooth sectioning.
  • Impaction patterns are described by the angle of the tooth against the second molar — mesioangular, horizontal, vertical, or distoangular — and by the depth of the tooth in bone. CBCT cone beam CT imaging is used in selected cases to assess proximity of the lower third molar root to the inferior alveolar nerve before surgical removal, allowing the partner maxillofacial surgeon to plan the approach and discuss the risk profile with the patient.

Surgical Assessment and CBCT Review

Surgical Extraction Visit

Under local anaesthesia, with IV sedation available where the patient requests it or where multiple third molars are removed in a single visit, the partner surgeon raises a flap, removes overlying bone, sections the tooth where indicated, and delivers the tooth from the socket. Bone edges are smoothed and the surgical site is irrigated and closed with fine sutures. The full appointment typically completes within 30 to 60 minutes per tooth, longer for deeply impacted lower third molars requiring careful nerve-aware dissection.

Recovery and Post-Operative Review

7–10 days typical

Soft-tissue healing typically takes seven to ten days, during which swelling, mild bruising, and limited mouth opening are managed with prescribed analgesics, a chlorhexidine rinse protocol, ice application for the first 48 hours, and a soft diet. Sutures are removed or self-resorb at seven to ten days. Full bone healing of the extraction socket completes over three to four weeks.
